Iain Rennie's 5 Year Vision
The Iain Rennie Hospice at Home believes that everyone has the right to receive excellent care in the place of their choice at the end of life. We aim to support patients, and their families and carers, to make it possible for all patients in our area to have the choice to be cared for and, if they wish, to die at home.
We want to be sure that everyone who could benefit from our services has these choices within our current geographical area by the end of 2011. We will then work towards establishing the Iain Rennie model of care for patients throughout Aylesbury Vale by the end of 2014. |
