Referral Process for GPs and other Health Professionals

The IRHH accepts referrals on behalf of any patient regardless of age, gender, sexuality, ethnicity and disability, and with any diagnosis.

The hospice will accept patients in line with the following criteria:

1. The patient has specialist palliative/complex care nursing needs
2. The needs of the patient and family/carer are such that additional supportive care is required, including end of life care
3. The patient wishes to remain at home for as long as possible
4. The GP consents to the referral
5. The patient consents to the referral, or the referral is made in the patient's best interest


How to make a referral

  You will be able to speak to a member of the office staff.  Basic patient details may be relayed at this point but referrals and further information can only be accepted by an IRHH Team Leader/senior nurse who will be asked to contact you as soon as possible. Out of hours referrals will be directed by our answering service and a nurse will call you back.
 
  • We would ask that the IRHH service is not committed to patients before the referral has been accepted.
  • When the referral is accepted an arrangement will be made for an IRHH visit and assessment.  We are always happy to make the first visit with a District Nurse or another member of the multidisciplinary team
  • A referral form signed by the referring professional needs to be sent to our office.  We will obtain a GP countersignature as this is our authorisation to work with patients.

  • The District Nursing service will be informed of the referral to IRHH.
  • When the referral is accepted, an arrangement will be made for an IRHH visit and assessment within 7 working days or sooner. A first visit will be carried out within one working day for an urgent referral. A non-urgent referral will be responded to within one week or at request of referring professional.
