Patients are normally referred to Iain Rennie Hospice at Home by their GP or District Nurse, but anyone can contact the service directly.
On referral one of the team members will visit you at home to assess your needs. A plan of care is then developed jointly with the Primary Health Care team, you and your family.
The Iain Rennie nursing service offers you access to specialist nursing care and support 24 hours a day throughout the year. Our nurses work closely with the GP and District Nurse sharing the care that is required. All professionals involved will communicate regularly to discuss and plan treatment and care.
Mostly the nursing visits will be planned in advance but should you need to call the nurses if you are feeling unwell, need advice or have a problem, you can do this at any time.
Your Iain Rennie nurse will give you a card containing information about the team involved in your care. You can access the service by telephoning the number on the card.
During office hours (9am - 5pm) your call will be handled by our office staff, who will contact a nurse in your team. All nurses have a mobile phone or pager, so can be contacted very quickly if they are out visiting other patients.
Out of office hours your call will be handled by staff at our answering service who will contact one of the nurses on your behalf. The nurse will visit you, or telephone you, whichever is deemed appropriate at the time.
There is no charge to the patient or their family for the nursing care we provide.
